Membership Application
If you would like to become a member of the Greater Phoenix Chamber of Commerce at Anthem North Gateway, please complete the form below. Once your form is submitted, Debbie Drotar will contact you for payment arrangements. The membership fee starts at $355 per year and is calculated based on the number of employees a business has.
Please note that all fields are required, unless specifically designated. If you would like to fill out a hard copy application, please contact Debbie Drotar at (602) 495-6483 and she will gladly send you one.
